Here’s the main points:

Dimensions:
Facebook Banner – 485 x 68 px
4 Vertical Guides @ these intervals (97 px, 194 px, 291 px, 388 px)
5 Small New Images for upload – 97 x 68 px

1. Create Banner
2. Merge Visible so that your art is 1 layer
3. Add the 4 New Guides
4. Create 5 new files (97 x 68 px)
5. Using marquee selector tool, select each slice, CTRL+C (copy) and CTRL+V (paste) each section to one of the 5 new images.
6. Save all as JPEG.
7. Upload to Facebook.
8. Tag yourself in each pictures STARTING WITH THE LAST IMAGE IN THE BANNER. Facebook works in reverse order when showing the banner according to tagging.
